We were out for the holiday.
It's a security error. The account impersonating AccuWeb.Accuwebserver doesn't have enough credentials to create it.
Check installation manual and set identity in DCOM configuration (or use AccuWebConfiguration tool).

I found the above helpful hint in another thread. Unfortunately when I click on the accuwebconfig.exe tool I get an error message about being unable to locate a visual fox pro support library.

Our users are unable to sign in.

That link was somewhat useful. Unfortunately the screen shots came with no directions and after a certain point I wasn't seeing the same things on my server as you were showing.

Regardless, this isn't an initial setup inquiry. Things were working great a couple of weeks ago and now they aren't.
