Activate check box was cleared for all Activities under all Categories, semester date range is correct and database has been repaired. any ideas?

Yes. In the Setup->Center Setup->Semesters screen, double .click on the current semester to edit it, then click the >> button to move all to "Selected", and then click "Save"
