It depends on the survey and also on the version of AccuTrack you are using.
In AccuTrack Verison 11.05 go to setup, options, feedback tab and you will see the opiton to show the activity feedback survey and the tutor/staff/advisor feeback survey at either sign in or sign out....

GO to Setup Options, and on the Weblink tab, check your settings for allowing a forgotten password link. Do new students have to be activated by admin?
Also, you should be able to upload passwords into AccuTrack. They are CASE SENSITIVE so make sure they are typing whatever case you uploaded them in.

Try double checking all your steps for the import. (if they are red when you try to import, that means they are already in the database)
Try making sure all the activities are moved under “selected” in the semester setup screen.
Note that you must click edit, then move them over to the right and then click save. Otherwise they will move but they wont save.
Make sure when you go to Activities, you click on the category. If the activity shows up, make sure it says its active.
If it still doesnt show up let us know. I called and left you a voice mail this morning so you can call me back if you need to.

Try checking to make sure you didnt accidently switch labs. Go to expanded view, go to Setup, Center Setup, Local Lab ID and make sure you are on the correct Lab. If you dont use labs in AccuTrack you would want to be on the Default lab also known as the "ALL" lab.
